FORMS FOR REGISTERED UNITS - INTERNATIONAL
CONTINGENTS
First Priority - Your Contingent Roster
- As soon as the members of your contingent have been selected, enter the
requested information for each member of your contingent and any International
Service Team applicants who plan to travel with your contingent on the
International
Contingent Roster Form. Then save it and email it as an attachment
to international@micamporee.org.
This information will be used to help match your contingent to a host troop or
crew. In addition, the names of contingent members will be listed in
letters supporting visa applications for contingents from countries where U.S.
visas are required. The deadline for submission of this form is January
20, 2008, six months before the weekend MIC 2008 international contingents
will arrive in Michigan.
Second Priorities
-
- An Information Sheet for each member of your contingent
International Leader Information Sheet for adult leaders and international
service team members
International Scout Information Sheet for Scouts
Each Scout and leader who plans to travel with your contingent should
complete one of these information sheets. Your host troop will use
these information sheets to match the members of your contingent with host
families. If, after attaching photos to the information sheets, you
are able to scan them and email them as attachments to
international@micamporee.org,
that would be great. If not, please send them by airmail to MICHIGAN
INTERNATIONAL CAMPOREE 2008, PO BOX 224, ST JOHNS MI 48879, USA.
- Travel Itinerary - Once you have completed your travel plans and have
confirmed reservations, complete the
Travel Itinerary Form, save it, and
email it as an attachment to
international@micamporee.org.
- Additional Forms to email to us by May 1, 2008
- International Contingent Performance – On Tuesday, July 22, at the Lake
Lansing Park South Bandshell and again on Tuesday evening, July 29, on the
outdoor stage at the Northwoods Campfire Bowl, MIC 2008 will hold
International Nights. Each international contingent is invited to
perform a song, skit, dance or other entertainment those evenings.
Your selection should demonstrate the culture or Scout association that you
represent. Whether it is a song that is always sung at camp in your
country or a dance from a specific region, we would like to include it in
the program. Please let us know your plans by completing the
International Performance Form, saving it, and emailing it as an attachment
to
international@micamporee.org by May 1, 2008.
- International Festival Day - On Saturday afternoon, August 2, MIC 2008
will hold an International Festival. The festival will take place at
the activity field (about the size of a football/soccer field). Every
troop at the camporee and many camporee visitors will attend. Each
international contingent and some patrols from the US will have a display
area to use and may choose to demonstrate an activity, share sample foods,
and distribute information or souvenirs from their country or region.
(All items will be offered for free. There will be no opportunity to
sell things.) There will also be an opportunity to repeat your
International Contingent Performance. Please use the
International
Festival Day Form to let us know what your contingent plans to do and what
supplies you will need, save it, and email it as an attachment to
international@micamporee.org
by May 1, 2008.
(The international contingents in your MIC campsite will also plan and cook
an international meal for your campsite troop and guests on Thursday
evening, July 31. Plans for that meal will be coordinated by your host
troop(s).)
- Forms you will need to bring with you (These forms are .pdf files.
If there is a space for “Council” please replace that with “Country.”)
- Health History and Physical Examination Forms - BSA requires Health
History and Physical Examination forms for all youth and adults who
participate in a week-long camp. There are two forms, the
Adult Health
and Medical Record Form and the
Youth Health and Medical Record Form.
Youth and adults who will be under 40 at the time of the camporee must have
had a physical examination, with a doctor’s signature, within 3 years of the
event. Adults who will be 40 or older at the time of the camporee must
have had a physical examination, with a doctor’s signature, within 1 year of
the event. We realize that in some countries this represents a
significant expense and hope that this advance notice will allow you to work
out group prices. Please make photocopies of these forms and bring two
sets with you. One set you will keep with you during home stay week.
The other set we will collect from you when you arrive in Michigan. It
will be given to our MIC Health and Safety staff so they can prescreen the
participants before opening day at MIC.
- Camper Medications Information - Any MIC youth participant who will be
bringing medicine to MIC and expects to take the medicine during MIC week
needs to have a parent complete the Camper Medication Form. You need
to bring those forms with you.
- Media Release - We need parental permission for Scouts who participate
in MIC 2008 to appear in photos, videos, etc., of the event. Adult
campsite troop leaders and staff members should fill out their own form, the
Adult Media Release Form.
For Scouts and Venturers, there are two options. If you would like
each parent to fill out a separate form, there is an
Individual Scout/Venturer
Media Release Form. If you would prefer to use one form for all
the Scouts in your contingent, and have all their parents sign that one
form, there is the
Group Scout/Venturer Media Release Form. Please choose one or the
other, collect signatures, and bring the adult and Scout/Venturer media
release forms with you.
- Swimming Ability Check - BSA rules require that all youth and adults who
plan to take part in MIC 2008 aquatic activities have their swimming ability
checked within 6 months of the event and be placed in one of three ability
groups, nonswimmers, beginners, and swimmers.
Nonswimmers have not passed a swimming test.
Beginners must pass this test: Jump feet first into water over the head in
depth. Level off and swim 25 feet (8 meters) on the surface.
Stop, turn sharply, resume swimming as before, and return to the starting
place.
Swimmers must pass this test: Jump feet first into water over the head in
depth. Level off and swim 75 yards (69 meters) in a strong manner using one
or more of the following strokes: sidestroke, breaststroke, trudgen, or
crawl; then swim 25 yards (23 meters) using an easy resting backstroke.
The 100 yards (92 meters) must be completed in one swim without stops and
must include at least one sharp turn. After completing the swim, rest
by floating.
If you can find a swimming instructor or lifeguard who will administer this
test and complete the Swimming Pre-test Form for the members of your
contingent before you leave for Michigan, so you can bring the form with
you, that would be very helpful. If not, we will try to arrange swim
tests for members of your contingent during home stay week.
